£43.53 per hour We are seeking an experienced and dedicated Advanced Practitioner to join our Transitions Team based in Nottinghamshire. Primary Responsibilities as an Advanced Practitioner: Provide high-quality support and guidance to young adults transitioning from children’s services to adult services. Work collaboratively with multi-disciplinary teams to ensure a smooth transition process. Conduct assessments and develop personalized care plans. Monitor and review the effectiveness of care plans and make necessary adjustments.
