An exciting opportunity has arisen for an Advanced Practitioner to join our developing multidisciplinary team at Arc Bucks Primary Care Network (PCN). Arc Bucks PCN is looking for an experienced practitioner to help deliver and develop services to patients in our care homes and provide care for minor illness, acute reactive care and proactive care to support our member practices. Our multi-disciplinary team is currently delivering Enhanced Health in Care Homes, key aspects of the PCN DES as well as working to meet the requirements and targets of the ICBs Care Homes Supplementary Network Service (SNS). Improvement in access to care is a priority for PCNs and this post will play a key role in increasing capacity providing care for minor illness, acute reactive care and proactive care in practice settings. We are looking for a PCN Advanced Practitioner who can provide high-quality, evidence-based and patient-centred care to the patients of Arc Bucks PCN. You could be an Advanced Practitioner, Pharmacist, or Paramedic. You will be responsible for triaging and seeing patients in our GP surgeries, care homes or in patients' own homes. Patients may present with a wide range of minor illnesses, urgent or routine problems. Main duties of the job

The role will include chronic disease management and require undertaking assessments including initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care working within defined procedures and protocols set by the PCN. You will need to demonstrate safe clinical decision-making and expert care, including assessment and diagnostic skills. There will be the need to prioritise and triage the needs of patients accordingly making any necessary referrals for investigations in the appropriate manner. As the PCN AP, you would provide expertise in: providing first point of contact direct access for patients of all ages with undifferentiated and undiagnosed problems seeking a primary health care service including on-call, telephone triage, face-to-face consultations, and home visits. Managing some on-the-day acute activity in practice/at home running PCN Wide Clinics - To assess and manage patients with undifferentiated and undiagnosed health care needs including investigating, treating, and referring patients as appropriate. Prescribe and review appropriate medications in accordance with local policies and evidence-based medical practice and work collaboratively with the wider team to support patients and to promote medication adherence. Primary Duties and Areas of Responsibility To provide a first point of contact for patients with undifferentiated and undiagnosed health needs. Provides and receives highly complex information develops communication skills/present complex information. To provide advanced assessment skills in history taking and clinical examination to formulate a diagnosis and initiate an appropriate plan of care, including referral to other health and social care professionals, as necessary. Instigate and interpret diagnostic investigations as appropriate. Provide patient and carer information, health education, and health promotion as appropriate. Prescribe appropriate medication and preparations within existing legislative framework in relation to evidence-based practice, NICE guidelines and considering the financial implications of cost-effective prescribing. Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and within scope of practice. Work with patients to support compliance and adherence to prescribed treatments. Demonstrate the ability to make professionally autonomous decisions when seeing and assessing patients with undifferentiated and undiagnosed problems, based on knowledge and skills, including skills such as physical examination, applied pharmacology and evidence-based prescribing. Work with all GPs and other members of the practice as directed in developing protocols for diagnosing and managing minor illnesses, and for chronic disease management etc. Work with the nursing, clinical pharmacy, and medical team in providing pro-active management of patients with chronic diseases in accordance with NSF and local requirements. Proactively identify, diagnose, and manage treatment plans for patients at risk of developing a long-term condition (as appropriate). Manage a caseload of complex and frail patients. Help support chronic disease management of a PCN defined cohort. Person Specification

Experience

Working at a masters level or equivalent that encompasses the four pillars of clinical practices, leadership and management, education and research with demonstration of core capabilities and area specific competencies. Graduated from a Centre for Advancing Practice accredited MSc advanced practice programme or completed Centres ePortfolio supported route allowing eligibility for an Advanced digital badge issued by the Centre. Mentor teaching qualification. Qualifications

Masters degree level in the relevant area of expertise. Skills and Knowledge
