JOB-20241107-791d94d2 Sanctuary Personnel, an innovative and committed recruitment agency has a new permanent position available for an Advanced Social Work Practitioner within the Hospital to work full time based in Hillingdon. The salary for this permanent Advanced Social Work Practitioner job is up to £54,129 per annum.

Main duties: To act as Duty Manager. To manage daily discharges via bridging pathway and pathway 3 placements. To triage cases alongside the Hillingdon HUB and out of borough hospital referrals. Attend conference calls with health professionals and deliver results. Support team members, allocate cases, and manage stepdown beds. Safeguarding Manager role, attending regular meetings with safeguarding lead at THH on progression of investigations. Carry a small caseload of complex cases. Support hospital discharges for our residents from Hillingdon Hospital as well as out of borough hospitals, rotating on a duty rota. Commit to providing quality services that promote independence/wellbeing, control, choice, and social inclusion. Demonstrate a person-centred approach by keeping the service user at the centre of the assessment and discharge process. Benefits: Free parking (4 days a week) if based at the Civic Centre.
