The post holder will: Work collaboratively with Shropshire Community Health Trust to support the patients within the rehabilitation and recovery setting. Be a highly specialised clinical pharmacist delivering a high level of clinical pharmacy input for those patients within the rehabilitation and recovery setting. Work as an independent prescriber to optimise medicine regimes for those patients within the rehabilitation and recovery setting. Be the lead pharmacist regarding the day-to-day delivery of a consistently high clinical pharmacy service for those patients within the rehabilitation and recovery setting. The service will be delivered in conjunction with junior pharmacists and medicine management Pharmacy Technicians and will include daily ward visits, participation in consultant ward rounds, and teaching of medical, nursing, and less experienced pharmacy staff. Lead the development of Clinical Pharmacy standards across the Trust for those patients within the rehabilitation and recovery setting. Be a clinical co-ordinator or tutor for the Keele University Diploma in Clinical Pharmacy. Working for your organisation
