Job Summary

An exceptional opportunity to lead a Local Cluster Collaborative (LCC) has arisen in Afan (Neath Port Talbot).

The Cluster Lead will provide senior leadership for the LCC, which comprises a wide range of partners including Social Services, the Third Sector, Community Nursing, Allied Health Professionals & Health Sciences, Mental Health Services and primary care contractors including General Medical, Dental, Pharmaceutical and Ophthalmic services.

Strong and inspiring leadership, bringing the wide range of LCC members together to translate national strategic direction into real action is required in this high profile role.

Expressions of interest from candidates are welcome from a range of backgrounds. The position is for two half-day/sessions per week or 0.2 WTE, remuneration is equivalent of Band 8c (A4C) or GP/dental sessional rate.

Afan LCC has a population of over 50,000 registered patients and serves a range of demographics supported by strong multi-disciplinary teams within the cluster.

Local knowledge of the Afan LCC area along with its population needs are desirable for this post.

Main duties of the job

The Role Will Report To The Clinical Director For Primary, Community And Therapies Service Delivery Group. Responsibilities Include Working Closely With Health Board Officers And LCC Partners To:

Remodel care pathways Deliver services within the community at LCC level Support and lead clear plans to improve population health, including playing a key role in steering the direction of the Pan Cluster Plan. Helping inform the future direction and maximise opportunities for the role of LCCs in the health board region Chair LCC meetings bringing together key stakeholders across the LCC area Support the development and delivery of effective clinical governance systems for the LCC in line with Swansea Bay University Health Board frameworks and policies. Lead the LCC to undertake an annual planning cycle integrated with the Health Board Strategic delivery plans, which is deliverable over three years Identify innovative programmes of work and oversee their successful implementation Ensure stewardship of substantial LCC resources, in line with Health Board Standing Financial Instructions Direction and line management of LCC Multi-Disciplinary staff and Business Implementation Manager.

Qualifications Essential Masters degree or equivalent experience at senior management level within the NHS Extensive recent experience in senior managerial role in Primary /Community Services Evidence of continuing professional development Excellent knowledge of a Healthier Wales and the vision and strategic direction of Primary Care LCCs Excellent knowledge of the new Primary Care Model for Wales

Desirable Health service management leadership qualification.

Experience Essential Demonstrable experience in delivering service improvements across primary and community healthcare setting Experience of leading on organisational and strategic development Experience of recruitment, workforce development and training Demonstrate evidence of sound financial/staff management Demonstrable experience of successful influencing and negotiating skills Strong networker and relationship builder
