Responsibilities:

Manage team Aged Care Needs Assessments Key Performance Indicators as contractually directed. Lead the Assessor team to deliver high quality in-person and remote comprehensive assessments. Develop and monitor strategies for assessors to connect people with appropriate services and supports in their community to further empower the client with their independence. Engage in peer review processes with triage and assessment delegates and assessors. Assign, un-assign, and reassign referrals for triage and assessments. Provide ongoing support and mentorship to assessors in the field. Essential Requirements: Tertiary qualifications in a health-related discipline (e.g., Medical Officer, Registered Nurse, Social Worker, Occupational Therapist, Physiotherapist, or other Health Professional). At least two years of experience in a leadership or supervisory role in aged care or a related field. Sound knowledge of Home Care Packages, Short term Restorative Care Program, Transition Care Program and Residential Respite Services. Previous experience performing in a KPI environment to ensure quality client outcomes. At least one year of experience in a related role is preferred. Unrestricted Australian Drivers Licence. National Police Check.
