As a Support Worker in our Community Aged Care Program, you’ll help clients stay independent and live comfortably in their own homes for as long as possible. Your role will be centred around empowering clients through a wellness framework, ensuring that everyone receives the care, respect, and dignity they deserve. You’ll work alongside clients to uphold our values and make sure our program’s goals come to life with compassion. Here is what you’ll be doing… Follow client support plans, ensuring that the care they receive is tailored to their needs. Help clients to meet their goals & outcomes. Build a supportive & engagement relationship with clients, enabling them to access social opportunities. Foster & promote a collaborative work environment. Record & report client changes to coordinators. Enact Rise’s values of Optimism, Welcoming, Respect & Integrity. This position is part-time, working Monday to Friday from 8:00am to 1:00pm. You will be required to travel to multiple client locations in your assigned metro area. Please only apply for this role if you can commit long term to the shifts & locations. Here at Rise, we nurture potential, but we do need you to have a few tricks up your sleeve including the following: A qualification in Individual Support - Aged Care, or equivalent (desirable). Ability to collaborate well with team members. Demonstrated ability to follow routines and procedures. Well-developed interpersonal & communication skills. A passion for working with people who are aged and/or have disabilities. Ability to work autonomously, while travelling to various client locations. Formalities (The serious stuff!) Appointments to this position will be subject to a National Police Clearance / requires a driving licence and reliable vehicle to use for work purposes / requires a pre-employment medical assessment.
