Job Overview Are you a registered children's nurse or learning disability nurse with relevant children and young person experience? Would you like to work in a flexible and friendly team during core hours Monday to Friday? We do not work any bank holidays within the year, including Christmas!

An exciting opportunity has arisen for a motivated and compassionate registered Children’s or Learning Disability Nurse to join All-age Continuing Care. A team driven to transform current practice and make positive changes to children, adults, and their families.

The post holder needs to be proactive, resilient, and able to work autonomously as well as part of the wider All-age Continuing Care Team. They will ensure that healthcare needs of both children and adults eligible for NHS Continuing Care or NHS Continuing Health Care are identified and met in accordance with the National Frameworks.

Main Duties of the Job

Support the clinical lead nurse and head of Continuing Healthcare in developing services for children and adults with complex health needs, including learning disabilities and Autism.

Work autonomously within a multi-agency setting.

Collate and undertake comprehensive assessments for decision-making for eligibility for All-age Continuing Care.

Support young people and their families through transitions to adult services.

Case manage All-age continuing care packages, ensuring timely reviews are conducted.

Develop strong working relationships with partner agencies.

Act as an advocate for patients and carers in partnership with local authorities and healthcare providers.

Person Specification

Active UK Nursing registration (child or learning disabilities).

Evidence of continued professional development.

Knowledge of safeguarding processes and responsibilities.

Strong clinical knowledge base for comprehensive holistic assessment.

Demonstrated capability to manage own workload and make informed decisions.

Good negotiation and conflict management skills.

Knowledge of national policies and guidance relevant to Continuing Care.
