About the Role
We are looking for someone to join our team in supporting adults of working age who face a variety of issues due to their mental health. The successful candidate will join an integrated service alongside both health and social care professionals. The role includes direct contact with service users, carers, support providers and other health and social care agencies.
The local authority has a responsibility under the Care Act (2014) to assess and meet the eligible social care needs of our fellow citizens. In this role, you would be reviewing any such care and support that’s been commissioned, always with the goal of maximising that person’s independence.
