Job summary To work as a member for the Children's Nursing Team, providing care for children and young people with complex or acute health needs within their care setting. Main duties of the job To manage their own caseload, waiting list and recording data onto the online system To plan and prioritise care activities taking into account patient needs and individual circumstances, ensuring all activity is recorded on the Child or Young Person's (CYP) health record To complete delegated observations and assessments of the CYP as agreed by the nursing team To develop, implement and evaluate appropriate care plans and comprehensive packages of care To provide quality care to Children and Young People in partnership with families and carers as commissioned and described within the service specifications About us A wonderful opportunity has arisen for an enthusiastic practitioner to work within the Children's Continence Team (0-18 years old). The job is for hours, and Monday - Friday hours can be arranged. There will be travelling expected around Norfolk to different areas to see children at home or in school. Most patients will be seen in a clinic environment, and you will have support from the Clinical Nurses within the team. Job responsibilities To use a broad range of communication skills to establish a rapport and engage with children and their families; to develop the child's and family's understanding of their needs and encourage effective participation in treatment To work independently with Children and Young People, individually and in groups, planning, organising and following specific programmes and guidance; to monitor progress and referring to the clinical nurse specialist where appropriate To work proactively to contribute to the Education Health Care Plan (EHCP) process To be responsible for completing own electronic diary and for booking own appointments To be aware of Norfolk Safeguarding Children's Board (NSCB) guidelines and procedures in the identification, assessment, and referral of vulnerable children Qualifications Essential NVQ 3 or equivalent Knowledge or experience Diploma level or equivalent knowledge and skills acquired through training and experience Desirable Nursery Nurse Qualification Knowledge or practical childcare Knowledge of play Knowledge of child development Care Certificate Experience Essential Experience of working with a variety of children Desirable Experience of working with children with disabilities and or working in children's own homes Skills, Abilities and Knowledge Essential Ability to work with indirect supervision of co-ordinator Good organisational skills Ability to remain calm and work effectively in pressure situations Basic IT skills Willingness to undertake training and development appropriate to the needs of the post Desirable Experience of working independently Communication Essential Good communication skills with children's families and professionals Use of English language Desirable Experience of using Signalong or Makaton Knowledge of alternative forms of communication Experience of working with people with sensory impairments and or communication difficulty Personal and People Development Essential Evidence of learning and development Personal Attributes/Behaviours (linked to the Trust's Behaviour Framework) Essential Non judgemental Professional appearance and behaviour Resilient, self-motivated and open to change Adaptable and able to prioritise according to changes in situation Enthusiastic and conscientious Able to identify with the Trust's commitment to safeguarding and promoting the welfare of children and young people Other Essential Must hold full and valid driving licence and have access to a vehicle Able to communicate effectively in written and verbal English Language Able to make own travelling arrangements to meet the needs of the service
