Job Purpose:

Provide support to the: Completion of holistic continence assessments and reassessments, Delivery of individualised management and treatment plans as delegated by the Community Continence Registered Nurse, Evaluation of the effectiveness for adult clients within a community setting, Provision of continence advice to clients, carers and those who work with clients with continence problems as advised by the Community Continence Registered Nurse, Maintain contemporaneous records as an autonomous practitioner working with supervision. Dimensions:

Ensure team are aware of own role and any new information for cover arrangements or support. To actively participate in CityCare mandatory training and team specific training. Key Responsibilities:

CLINICAL To support the completion of a holistic continence assessment and delivery of individualised plan of care, under the supervision of a Registered Community Nurse Continence Care. To escalate clients to the Registered Community Nurse Continence Care when their needs are beyond the Assistant Practitioner (AP) role. To accept responsibility for designated caseload, working independently taking an active role in promoting independence. To promote health and wellbeing and effective communication. To provide instruction, advice and intervention to clients and carers in line with the care plans, policies and Standard Operating Procedures. To attend and contribute to team meetings and case discussions using effective communication. To maintain comprehensive and accurate records in line with employers policies. To undertake a range of nursing competencies, as per the AP competency framework. To operate under the guidance of clinicians and only within the scope of AP practice. MANAGERIAL AND LEADERSHIP To provide support to other Assistant Practitioners as appropriate. Act as professional role model aligned with Trust values and behaviours. EDUCATION AND DEVELOPMENT To ensure knowledge, skills and competencies are kept up to date and complete all relevant competencies within recognised time frames (as detailed in the competency guidelines). To be involved in formal appraisal systems, supervision, and be active in identifying aims and objectives for personal development in line with departmental policy. Also, to keep an active up to date CPD portfolio. CLINICAL QUALITY AND GOVERNANCE To maintain a safe working environment and be aware of the necessity to comply with health and safety regulations. Support and engage with processes relating to staff and patient safety to align with the Patient Safety Incident Response Framework (PSIRF). Support and engage with processes relating to staff and patient experience as appropriate. Support and engage with processes relating to clinical audit and data collection as appropriate. To always have regard for the confidential nature of the work by adhering to confidentiality legislation and not to disclose information to unauthorised parties. To actively promote equality and diversity within service delivery and peoples individual rights. The post holder will comply with current legislation, the organisations policies, procedures, and core values.
