BSW Continuing Healthcare (CHC) Service is responsible for ensuring that patients referred for NHS Continuing Healthcare and NHS Funded Nursing Care (FNC) funding receive a high-quality assessment of need, ensuring that the person is at the heart of the assessment, that their view and those of their family and carers are taken into account and that all assessments are completed within national timescales. The post holder will assist in the implementation and delivery of the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care (revised 2022) and be able to apply it within the context of the assessment role. The Assistant Practitioner for CHC/FNC will work under the supervision and direction of CHC Case Coordinators. Main duties of the job

The role will include completing CHC Checklists on individuals in care homes without direct support of senior staff. You will be responsible for providing relevant and timely advice, guidance and information on matters relating to the CHC/FNC process in line with your role. You will have excellent communication skills and possess insight into non-verbal communication. You will have experience in working within a multidisciplinary team, which includes carers and or family members and will ensure CHC Checklist assessments are fully inclusive. Job responsibilities

Working within the NHS Continuing Healthcare Team to support the assessments of the Funded Nursing Care contribution to individuals who reside in care homes with nursing. Using knowledge and clinical experiences to ensure that assessments and reviews are determined in a consistent manner to ensure effective application and interpretation of the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care. Qualifications

Diploma in health or care related subject or NVQ level 3 in health or care related subject with post qualification evidence of further study / additional training to diploma level or NVQ 4 in Health or equivalent level experience or Evidence of extensive and current paid experience of working with service users health and care needs. Educated to GCSE level or equivalent (with a minimum of 5 A - E qualifications - which must include Maths and English). Knowledge

Experience and awareness of NHS Continuing Healthcare. Demonstrated experience in a Healthcare environment. Skills Capabilities and Aptitudes

Clear communicator with excellent written and presentation skills. Proven interpersonal skills to work with staff at all levels as well as individuals and/or their representatives. Ability to manage own time and prioritisation of own workload. Working collaboratively with a range of colleagues across health and social care professions. Resilience to deal with emotive and contentious issues and confrontation. Full driving license and access to a car. Ability to spend periods of time in front of a monitor. Values and Behaviours
