Assistant Practitioner Integrated Transfer of Care (ITOC) Hours:

21 hours per week Type:

Permanent Locala ITOC provides comprehensive care for adults in their own homes, community rehabilitation facilities, and care homes. We have an excellent opportunity for an experienced Assistant Practitioner to join our team. The role is based across North and South Kirklees, requiring flexibility in terms of service needs, with an expectation of travel across Kirklees. The Assistant Practitioner will provide rehabilitation and support within the ITOC team in various settings. This role requires a motivated and dynamic person to work within our established service, and it is essential to have experience in working as part of a multidisciplinary team. Experience in treating patients with complex disability, orthopaedic conditions, delirium dementia, and falls is required. The ability to be flexible and work in a variety of unplanned care settings (e.g., A&E, with frail patients in the community, and discharge support in the Acute Hospital and Recovery hub) is essential. A formal Assistant Practitioner qualification is desirable, but a willingness to work towards this is essential. Benefits
