We are looking for enthusiastic, motivated individuals to join our expanding Prestwich District Nursing Team based in a community setting. This is an exciting opportunity to be involved in the integrated health and social care organization. You should have excellent communication and interpersonal skills which will be utilized within this multi-disciplinary health care team. You must be multi-skilled and be able to work across various disciplines. You must be proficient in providing planned care and be able to take appropriate action in achieving set goals with minimum supervision. Main duties of the job

You will carry out screening/therapy/nursing procedures as identified within your role. The Assistant Practitioner / Nursing Associate will provide supervision and assessment for Healthcare Support Workers undergoing relevant training. You will be given a dedicated induction programme and a full training and development plan will be offered. You will have an NVQ level 3, a formal Assistant Practitioner qualification, and be practicing as an Assistant Practitioner/ Nursing Associate. We welcome forward-thinking individuals who would like to be part of a dynamic rapid response team covering Bury. Due to the nature of the post, it is expected that you will have a full driving licence and will have regular use of an appropriate vehicle (as determined by the Trust). Qualifications

Assistant Practitioner Qualification Health and Social Care (Assistant Practitioner) FdSc or Health and Social Care Foundation Degree (FdA) or equivalent Evidence of continuous professional development (CPD) Experience of working with patients and members of the public in a caring environment Level 2 Literacy and Numeracy (GCSE English and Maths Grade A-C) Evidence of involvement in teaching Experience of undertaking clinical observations and procedures, interpreting the results and escalating appropriately Experience
