Description: Full or Part-time Performer Dentist required for 2-5 days per week in a busy, mixed, well established and INDEPENDENTLY OWNED practice near Hornsea, East Riding of Yorkshire. • £15 per UDA (negotiable UDA contract) • EXCELLENT PVT POTENTIAL (NO LONGER ACCEPTING NHS PATIENTS ONLY PVT) • 50% split on private & labs • Established list • Fully computerised surgeries - Hygienist on-site • Large 6 surgery practice • OPG • Digital x-rays • Rotary endo • Sponsorship available but need performer number as no mentorship • Short walk to station • Full support team including a hygienist • 20 minutes from Beverley and 40 minutes from Scunthorpe • Available Immediately, start date negotiable

Compliance requirements: - Full NHS Performer Number if applicable - Local Area Team confirmation - GDC certificate - UK Blood test report showing Hep B Immunisation and Titre levels, Hep C, HIV and TB results dated within last 5 years - Indemnity certificate - Passport - Enhanced DBS (within the last 12-36 months) - Clinical references covering places of employment over past 2 years (minimum 2 referees) - Degree certificate - CPR/AED/Medical Emergencies certificate dated within the last 12 months - IRMER - PIN number
