ROLE: Band 4 Health Care Assistant (Ref; R20) – Female Only Role TEAM: Redbridge Mental Health and Wellbeing Team South (GWN2) WORKING HOURS : Monday to Friday 9am to 5pm DURATION : 3 Months RESPONSIBILITIES : The post holder will support their peers and team leader and be an effective team member whilst working on their own initiative, ensuring people with Severe & Enduring Mental Health, take up annual health checks. Further referring onto specialist services where any anomalies arise. They will work closely with mental health professionals regarding Recovery principles to keep people in good physical health. They will have a commitment to collaborative working, actively supporting and liaising with other health and social care professionals, particularly Primary Care Network staff. Complete Health Screen for service user's with server mental illness, input and tranfer health screen records from NELFT RIO to GP data base (EMIS). Complete all admin duties for the clinic including booking and sending appointmnets, and follow up.
