Band 5 - Clinical Pharmacy Technician - BHRUT We provide care for a population of around 750,000 people across north east London (NEL) and that number is predicted to increase by 15 per cent over the next ten years. We serve three London boroughs with diverse populations, and more than half of our workforce identify as Black, Asian, or Minority Ethnic. In addition, eight out of every ten employees are women, and most of our workforce lives within the host boroughs of Barking and Dagenham, Havering, and Redbridge. Our services include all the major specialties of large acute hospitals, operating from two main sites - King George Hospital in Goodmayes and Queen’s Hospital in Romford. We also provide outpatient services at Brentwood Community Hospital, Barking Hospital, Loxford Polyclinic and Harold Wood Polyclinic. We have two of the busiest emergency departments in London. Hours of work: Monday to Friday 0900 - 1700; post may include weekends, late shifts, and Bank holidays. Location: Queens Hospital Clinical / operational responsibilities As part of the provision of patient/client care, the post holder will: Provide a regular pharmacy technician transcribing service to designated wards. Provide professional guidance and supervision to Junior Pharmacy Staff. Be responsible for accurate and timely transcribing of patients' discharge medicines onto the electronic discharge summary (eDS), liaising with prescribers to deal with any outstanding medicines reconciliation discrepancies or queries around the patients' medication regimens required for discharge. Be accredited to take on clinical pharmacy technician duties including ward CD cupboard checks, assessment of patient’s own drugs, medicines reconciliation and drug history taking, counselling patients on their medicines and discharge/in-patient transcribing. Provide patients with information about their medicines at any time during their admission in hospital, including informing the patient of any new medicines, changes to doses of their regular medicines, and any stopped medicines. Identify and provide medication counselling to patients who have been started on new medicines, including inhalers, novel oral anticoagulants (NOACS), or to patients who it is felt need extra information about their medicines to improve compliance and concordance. Ensure medicine discrepancies are acted upon, within the remit of the clinical pharmacy technician role, and record and share these with other members of the team to keep patient safety high on the agenda. Important Requirements: Registered as a UK Pharmacy Technician. Level 3 QCF Diploma in Pharmacy Service Skills. Level 3 QCF Diploma in Pharmaceutical Sciences. ACPT Checking Qualification. Previous hospital pharmaceutical experience - at least 6 months within the last 3 years. Previous experience with automated dispensing systems. An empathic, caring approach to patients, and a great team player. An honest individual with patience and integrity, with a willingness to learn and grow.
