An opportunity has arisen for a Band 5 Occupational Therapist to join the At Home Team in Sutton Health and Care. We are looking for candidates with the relevant knowledge and skills, and enthusiasm to deliver and develop the OT workforce within the Multidisciplinary team. You will be part of the MDT team that helps to prevent admission from community, accelerate discharge from emergency department and acute medical ward, Acute Frailty ward and provide supported discharge through Discharge to assess model of care through to community based Rehabilitation all within one team. Key responsibilities include: To deliver skilled occupational therapy services across the team. The post holder will have clinical responsibility for assessment and treatment planning for people referred. They will be expected to participate in developing and monitoring the service. To work closely with multidisciplinary teams and other professionals, particularly in relation to patient enablement, and to assist in the development of good working relationships with statutory, voluntary and independent organisations and agencies.
