It is essential that you are interested in and excited about the opportunity to work with older people – training will be provided. You will care for your patients holistically, involving carers and relatives and work alongside medical and therapies staff. You will be able to adapt quickly to changing situations and provide high quality care to your patients. Responsible for the assessment, implementation, and evaluation of care plans for individual service users on an Older Adult Inpatient Ward. Work as a key worker for individual patient needs. Assist the team in setting and maintaining high standards of care and evaluating quality outcomes. Participate in a range of assessments provided during a 24-hour period. Maintain good standards of communication, both oral and written, in accordance with NMC guidelines. Report to the nurse in charge (or nominated deputy) any circumstances that may affect the delivery of care and/or efficient running of the ward. Assist the ward in the day-to-day operation of the clinical area and the development of the team. Ensure that the environment is clean, pleasant, and comfortable and suitable for its use at all times, taking action as appropriate to ensure high standards are maintained. Contribute to the development of the service. Work collaboratively as a Key Worker within the multidisciplinary team, acting as a role model for good practice. Ensure that service user centred care plans are developed, implemented, evaluated, and recorded. Ensure effective communication with service users, relatives, visitors, and staff. Implement and ensure effective use of the Mental Health Act and Mental Capacity Act where necessary. Ensure safe standards apply to the administration of medication. Be an effective role model to all staff within the working environment. Attend mandatory courses. Participate in Clinical supervision. Support colleagues in interventions relating to the management of violence and aggression. Maintain effective relationships with service users and members of the Trust. Person Specification

Qualifications

RMN Qualified Band 5 Evidence of continued professional development. Experience

Demonstrate an understanding of patient and carers involvement in planning and delivery of services. Development skills and relevant qualification. Please note:
