Band 5/6 Community Mental Health Nurse - Bath
An exciting opportunity has arisen for a community mental health nurse (Care Co-ordinator) to join the later life Complex Intervention and Treatment Team (CITT) based in NHS House in Bath. The appointment could be made at a Band 5 or Band 6, depending on the experience of the applicant, and is therefore offering an exciting development opportunity. The team provides a range of key interventions which can include: The assessment, formulation, treatment & therapeutic support for older people with severe & enduring mental health problems, either with a functional illness or with dementia. Pharmacological interventions - medication management, prescribing and monitoring. Psychosocial and psychological interventions. Working in collaboration with AWP MDT and non AWP services. Complex organic and dementia presentations, including behavioural and psychological symptoms associated with dementia (BPSD). Statutory work under the Mental Health Act and Mental Capacity Act. Working in partnership to promote safeguarding of children and vulnerable adults and public protection. Recognition of carer's needs, signposting and help in accessing other services. Main duties of the job
Applicants will need to demonstrate knowledge in the areas of the delivery of mental health care in the community, with particular reference to the needs of Older People, together with an understanding of both the CPA and Care Management processes. You will be required to provide care co-ordination for an allocated caseload of service users with substantial and complex mental health needs. This will include key tasks of undertaking high quality assessments, developing care plans, delivering and reviewing therapeutic interventions. You will act as a supervisor and role model to others. Applicants will be expected to demonstrate a high level of decision making skills and to work independently. Applicants will also need to be proficient with the use of IT systems for both data and documentation purposes. Job responsibilities
To contribute to the full range of activities required to deliver ongoing comprehensive mental health assessment for service users with severe and enduring mental health needs living in the community and in a range of settings. Triage of new referrals made into secondary mental health services. Completion of high quality comprehensive mental health needs assessment, including risk assessment, for service users. Signposting of assessed individuals to appropriate services according to their needs. To contribute to planning, delivering and reviewing treatment programmes using appropriate frameworks in line with evidence-based practice, including strategies to manage risk for service users with complex needs and carers, bringing in other resources as required. To act as care coordinator for identified service users, also providing defined interventions to individuals on other workers caseloads. To develop and maintain good partnership working with other services throughout all treatment episodes. To personally build, hope inspiring relationships with service users, which acknowledge the personal journey of each person, and focus on strengths and aspirations to allow the creation of meaningful personal recovery plans. To maintain the single health and social care record, ensuring both paper and electronic records are kept up to date in accordance with professional and organisational standards. To provide mentoring/training for others in relevant practice areas, according to professional requirements. To adhere to professional codes of conduct ensuring required skills and competencies are maintained. Person Specification
Essential knowledge, skills and experience Diploma / Degree and registration with the NMC Demonstrates a good understanding of recovery principles and the role of secondary mental health services in the delivery of care Able to articulate a sound understanding of the relevant legal frameworks/legislation including CPA process, Mental Health Act, Mental Capacity Act, and Safeguarding. Demonstrates significant experience of assessing risk and developing risk management strategies Demonstrates post-registration experience gained working in front-line mental health services, undertaking assessment and delivering interventions. Demonstrates experience of supervising others, monitoring their performance and ensuring delivery of activity. Ability to prioritise, particularly when working under pressure Well developed verbal communication skills, able to engage effectively with people at all levels including when a more assertive approach is needed.
