An exciting opportunity has opened for a Band 6 Care Coordinator to join the NYYS AMH HHR Perinatal Team, working within the NHS in Northallerton. This role offers a blend of onsite and remote working in a hybrid model, giving you flexibility while making a real difference in the community. £25–£28 per hour The successful candidate will play a pivotal role in supporting individuals and families during the perinatal period. You will be part of a multidisciplinary team known for its compassionate, person-centered approach and its commitment to delivering innovative mental health care. Proven experience as a Care Coordinator. Expertise in mental health and the ability to work effectively within multidisciplinary teams. Qualification as a Nurse, Occupational Therapist (OT), or Social Worker (SW).
