We are seeking a skilled and motivated Band 6 Community Occupational Therapist to join a Community General Rehabilitation team in Dartford, Kent. This role involves working closely with patients in a general rehabilitation ward and conducting home visits to support their recovery and enhance their independence.
Responsibilities include, but are not limited to: Conducting comprehensive assessments to identify patients' physical, emotional, and social needs. Developing and implementing tailored rehabilitation programs to achieve individual goals. Providing advice and support to patients, families, and caregivers. Performing home visits to evaluate living environments and recommend adaptations as needed. Collaborating with a multidisciplinary team to ensure high-quality, patient-centred care. Maintaining accurate and timely documentation of patient progress and interventions.
Requirements: HCPC registration and a degree in Occupational Therapy. Demonstrated experience in general rehabilitation or similar roles. Must hold a valid driver's license and have access to a car for home visits. Enhanced DBS dated within the last 12 months. On the update service? Pay rates: Day Weekdays: £30.54 per hour Day Saturdays: £33.15 per hour Day Sundays & Bank Holidays: £35.76 per hour
