The main aim of the Community Rehab Service is to enable patients to work towards their own identified goals in their own environments. The post holder will be required to use enhanced knowledge and skills to assess and facilitate rehabilitation programmes with patients and their support network to maximise their daily abilities in their daily life and ongoing self-management skills. There will be a requirement for post holders to work closely with colleagues in the local neighbourhood and community services/teams. You will be expected to carry out specific therapy assessments and devise rehabilitation programmes. This will include treatment planning, assessing risk, promoting and enabling independent functioning. Maintains good communication, liaison and working arrangements with other Trust teams, Trusts and external agencies. To use communication tools (verbal and non-verbal) to gain valid informed consent with patients where there will often be barriers to effective communication e.g. loss of hearing, altered perception, expressive and receptive dysphasia, pain, and fear. To have the capacity to work within a legal framework with patients who lack capacity to consent to treatment e.g. those with cognitive or speech difficulties. Person Specification
Qualifications
Degree or equivalent in Physiotherapy Evidence of further relevant training qualifications. Experience
Qualified Physio experience in a wide range of settings and a range of diagnoses. Previous Band 6 experience Previous community rehab experience. Additional Criteria
Must be a car driver with a car available for work. Experience of Team working. Disclosure and Barring Service Check
