Band 6 Frailty Occupational Therapist - North Wales

A fantastic opportunity has become available for a Band 6 Occupational Therapist to join an Acute Frailty Team in North Wales. This role offers a competitive hourly rate of £30 to £38 per hour, based on experience, with both full-time and part-time options available. The successful candidate will start in January 2025. Full-time (open to part-time) The position will involve working within a fast-paced, multidisciplinary team, providing high-quality occupational therapy services to frail and elderly patients. The focus will be on acute frailty, supporting patients to regain independence, improve functionality, and enhance their quality of life during their hospital stay and beyond. Key Responsibilities: Assess, plan, and deliver occupational therapy interventions for frail patients with complex needs. Provide rehabilitation to enhance independence in daily activities and promote safe discharge planning. Work collaboratively with multidisciplinary teams to deliver comprehensive care plans. Educate and support patients and their families on managing frailty and improving functional outcomes. Maintain accurate and up-to-date patient records in accordance with trust guidelines. Contribute to service improvement initiatives within the frailty pathway. This role is ideal for an experienced Band 6 Occupational Therapist with a passion for frailty care and rehabilitation.
