An exciting opportunity has arisen for a forward-thinking dynamic Physiotherapist to join the Bedded Intermediate Care Units. Bolton Hospital NHS Foundation Trust and Bolton Council work in partnership to provide patient-focused care in residential units in the community. This role will involve working in our Intermediate care beds and Community Beds (Enhanced Health in Care home Team) - Discharge to Assess Beds to ensure the delivery of a safe and high-quality service.

Bolton Intermediate Care focuses on the prevention of unnecessary hospital admission, effective discharge from hospital, and promotion of independence, especially for the older person. We have a large and comprehensive multi-disciplinary team that cover 60 beds located across 2 residential sites in the Bolton area.

Main duties of the job The position we are recruiting to will be part of the Intermediate Care therapy team, consisting of Physiotherapists, Occupational Therapists, and Therapy Assistants. The successful candidate will be expected to rotate across two units and Discharge to assess beds provision to ensure the needs of the service are met. The successful candidate will work as part of a health and social care multidisciplinary team comprising medics, pharmacists, social workers, and nurses.

Candidates will have recent and relevant post-registration experience and an interest in elderly rehabilitation in an acute and community rehabilitation setting. They should also be able to evidence recent self-directed learning and development.

Candidates will need to be able to travel to meet the needs of this post and will therefore need access to a vehicle for work.

Person Specification Qualifications Essential Degree or equivalent in Physiotherapy Current professional registration as a Physiotherapist

Experience Essential Treatment of a broad range of conditions relating to clinical specialty Evidence of leading and implementing clinical change Supervision and/or mentoring of staff/students. Multi-disciplinary/multi-agency collaborative working.

Skills Essential Ability to communicate highly complex pathologies and diagnostic reasoning to all levels in the medical and non-medical field at the appropriate level. Able to undertake assessment in difficult complex situations Sound knowledge of psycho-social problems and ability to gain patient confidence and cooperation. Able to work independently as lead specialist, demonstrating good leadership skills.

Knowledge Essential Understanding of clinical governance framework and implications for practice including audit and research governance. Understanding of current NHS/government policy/Public Health/NICE guidelines on organisational change and the ability to adapt these appropriately to local service needs through publication of documents. Change management skills

Other Essential Ability to work flexibly and travel across the service footprint to meet business needs Able to work under pressure and constantly respond and adapt to frequent changes in circumstances.
