The post holder will work as part of a multidisciplinary primary healthcare team delivering primary healthcare nursing, including elements of occupational health within the Pirbright Combined Medical Practice. This will include being a focal point for Primary Health Care nursing issues within the area of practice. The Combined Practice is formed of 2 sites Pirbright and Windsor, and Networked with the Sandhurst Practice. There is an expectation the Band 6 nurse will be required to work across all 3 sites. Job responsibilities

Duties and responsibilities for the role include but are not limited to: Have direct responsibility for patient caseload, ensuring all patients have an accurate plan of care, which reflects the assessment undertaken and incorporates the issues and recommendations made. Manage the care of patients with chronic illnesses e.g. Asthma, Diabetes, Chronic Heart Disease (CHD) and hypertension ensuring continued maintenance of the Chronic Disease Register. Undertake technical nursing treatments and procedures for patients with complex/specialist healthcare needs (e.g. wound care, suture removal, Electrocardiograms (ECGs), Cytology, Audiometry, Ear Care). Undertake health and well-being clinics (e.g. smoking cessation, weight management, alcohol misuse) using health promotion, health education, screening and therapeutic communication skills. Registered Adult Nurse - on Part 1 of the UK NMC Register Experience

Experience of providing care to patients with chronic illnesses e.g. asthma, family planning, Diabetes, Chronic Heart Disease (CHD) and Hypertension ensuring continued maintenance of the Chronic Disease Register. Experience of having provided mentorship to other staff. Qualifications

Professional and specialist knowledge in primary healthcare acquired through post graduate degree/diploma or actively working towards this and can demonstrate equivalent level of competence and experience Learning and Assessing in Practice Qualification or equivalent practice assessors training Quality improvement methodology or willingness to undertake training Must be in date for Basic Life Support (BLS), Anaphylaxis, Automated External Defibrillation (AED), Infection Prevention and Control (IPC), Safeguarding Level 3 (Children and Adult depending on population at risk) or willingness to undertake training Registered non-medical prescriber or willingness to undertake training Evidence of continuous professional development and the practical application of new skills to the workplace. This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions. Salary: £37,338 to £44,962 a year, Per annum, Pro-rata for part time.
