The post holders will rotate every three months alternately between the following two areas, spending six months in each: Working across all ICHT sites to support the referral and onboarding of patients onto our Virtual Wards pathways. Working with our colleagues on the NWL Virtual Hospital based at St. Charles Hospital. The Virtual Wards Onboarding Nurses will provide appropriate support to the Virtual Wards Onboarding Nurse Coordinator, Business Manager, General Manager, Head of Specialties, and the Clinical Director in the delivery of all operational functions of Virtual Wards. This will include clinical management of patients on digital care pathways, as part of the NHS Long Term Plan emphasising digitally enabled care. The post holders will be responsible for receiving and reviewing referrals to Virtual Wards pathways and leading the onboarding process, supporting the patients with the processes and tools. They will also support case finding, reviewing and assessing appropriateness, liaising with Virtual ward teams and other clinical teams. The Onboarding Nurses will support the Clinical Lead and Heads of Pathway to provide leadership for the pathways and to further education and awareness of the Virtual Wards Programme amongst staff and patients. Main duties of the job

Key areas of responsibility Onboarding onto and care in Virtual Wards Effective use of resources Maintaining and improving standards of patient care Clinical Responsibilities Carry out planned care for a group of patients without direct supervision, utilising escalation pathways to ensure care is coordinated into organisations in North West London. Ensure the assessment, planning, implementation and evaluation of evidence-based, individualised patient care, including supporting case finding, reviewing referrals from teams and onboarding to Virtual Wards pathways. Assist and advise staff, both registered and unregistered in the delivery of patient care through the digital health record. Maintain accurate clinical observations of the patient and act accordingly. Have responsibility for the correct administration and custody of medicines according to Trust policy. Monitor patients' progress, work towards safe and timely discharge plans and ensure barriers to discharge are identified and acted on appropriately. Monitor caseloads in conjunction with specialist nurses and team members, raising concerns as appropriate. Act as the patients' advocate and troubleshoot any issues they raise. Assist the Digital lead Nurse in providing care in a digital remote context and deputise in their absence. Monitor standards of care and take appropriate action to maintain and improve them when monitoring patients remotely. Respond to patients, relatives and carers' concerns as they arise and take remedial action as required. Assist hospital teams in achieving a digital personal health record, helping to ensure the team are able to provide timely review/transfer/discharge of patients. Registered Nurse Relevant specialist course or equivalent experience in a relevant specialty Teaching Qualification or equivalent experience Disclosure and Barring Service Check
