Provide a ward-based clinical pharmacy serviceaccording to Trust agreed standards including: individual prescription reviewto optimise therapy, confirmation of the patients medication history, adviceon dosage, side-effects, cautions and monitoring required and offer duties. Participate in ward rounds, clinical meetings e.g.ward user group or out-patient clinics as appropriate in order to providepharmaceutical advice to prescribers and other health care professionals. Liaise with the rest of the members of the clinicalpharmacy team to support the strategic developments of the service e.g.electronic prescribing, improved discharge processes, accredited technicianchecking etc. Actively contribute to the planning and efficientco-ordination of clinical pharmacy initiatives including discharge planning,prescribing protocols, pharmacists on consultant ward rounds etc. Support the monitoring of prescribing and drugexpenditure within the clinical area and provide advice on cost containment. Qualifications

Member of General Pharmaceutical Council (GPhC). Qualified to Masters Degree level (MPharm or equivalent). Experience

Hospital experience is preferable but not essential. Disclosure and Barring Service Check
