Band 7 Advanced Mental Health Practitioner

The post holder will be joining a well-established team working with older adults (65+) admitted to Wexham Park Hospital. The team supports patients on all medical wards across the hospital. Patients are primarily presenting with mental health or cognitive difficulties in the context of declining physical health and/or neurological or neurodegenerative conditions. The nature of the work is varied and dynamic and there are many opportunities to develop close working relationships with specialist departments, including diabetes, heart failure, oncology, stroke, care of the elderly and palliative medicine. Main duties of the job

Undertake comprehensive initial assessments of the mental health needs of older people who are currently receiving treatment for physical health problems within a general hospital setting, taking into account their physical, psychological, social and spiritual needs. Plan, facilitate, deliver and evaluate specialist person-centred care and treatment for older people with mental health problems in the context of their physical health and current admission, and where appropriate include the views of their family and carers. Offer consultation and highly specialist advice to colleagues from the general hospital multi-disciplinary team, regarding appropriate treatment and management of older people who present with mental health problems in the context of declining physical health. Liaise effectively with other agencies providing services to older people with mental health needs, ensuring that all relevant personnel are aware of any changes in care/treatment or patient needs. Make appropriate onward community referrals for the continuing treatment of the mental health needs of patients and to put in place appropriate care plans to support ward staff with discharge from hospital. Experience

Experience of liaison psychiatry. Experience of working with older adults in mental health. Qualifications

Qualification in Mental Health Nursing. Registered Mental Nurse, Social Worker or Occupational Therapist. Disclosure and Barring Service Check
