As the result of the Service need to cover Maternity leave, here at West Middlesex Hospital and in particular the Rapid Access Chest Pain Clinic, we are looking to recruit a Senior Nurse with experience in this area to support the service. To work cross-site to standardise services and care for patients presenting with chest pain and to ensure that the service sees patients in a timely way, assisting the Trust to meet access targets. To support and contribute to the organisation's existing nurse led Rapid Access Chest Pain Clinics, adhering to protocols and guidance agreed by the Team's consultant cardiologists, basing decisions on professional knowledge and expertise, providing a one stop service for patients with new onset chest pain referred by GPs within the North West Sector catchment area. Main duties of the job

To provide expert assessment of patients with chest pain, to operate within protocols, utilising advanced nursing skills and knowledge to identify or refute coronary artery disease. Ordering investigations, interpreting results and reports, recommending appropriate pharmacotherapy, determining discharge or inpatient admission where appropriate and presenting findings to medical staff where appropriate. To utilise protocols and integrated care pathways for patients attending the clinic, to enable appropriate risk stratification and early revascularisation referral where appropriate. To maintain clinical audit of this client group, meeting the requirements of the National Service Framework for Coronary Heart Disease (2000) and NICE chest pain guidance 2016. To become an integral part of the cardiology team and to contribute to service development in other areas of cardiology within the Trust. To act as an educator, helping patients to understand and manage their symptoms and facilitate strategies for lifestyle modification when appropriate. About us

Clinical To support and contribute to the nurse led Rapid Access Chest Pain clinic service in accordance with NSF 2000 targets. To make an assessment of patients' cardiac care needs based on highly developed nursing knowledge and skills including advanced cardiac physical assessment skills and history taking and request appropriate investigations. To assess patients' risk of CVD using NICE guidance and empower patients to realise and maintain their potential in relation to health and wellbeing. To assess complex health and wellbeing needs, diagnose or refute diagnosis of Coronary artery disease using accountable professional decision making and problem solving skills autonomously. To develop with the patient a management plan for their cardiac needs within the remit of the clinic with emphasis on treatment including revascularisation where appropriate whilst emphasising health education and prevention measures. To assess and authorise the admission or discharge of patients from caseload as appropriate. To ensure patients and relatives are given appropriate oral and written information regarding Coronary artery disease. To provide health promotion to individuals and their family regarding Coronary artery disease and risk factors for primary and secondary prevention. To review and maintain standard of documentation in accordance with the NMC standards for records and record keeping. To act as a gateway for the GP referrals ensuring that appropriate referrals are accepted to the RACPC and inappropriate referrals are forwarded to a consultant cardiologist. To inform GPs of clinic outcome and requested investigations by letter. Realise their own limitation of ability and recognise boundaries and consult with other members of the team as appropriate. To develop a culture that improves quality within the Rapid Access Chest Pain service. To be an independent nurse prescriber in accordance with nursing and midwifery council (NMC) standards for prescribing medicines and trust policy. Person Specification
