Job summary

The Palliative Care Clinical Practitioner (PCCP) will support delivery of palliative and end of life care, acting as the generalist 'expert' for the locality and working collaboratively across all community services to lead and influence continuous improvement in palliative and end of life care. The post holder will support teams deliver the 'home first' approach, working in partnership with discharge teams/hubs across acute and community services, EICTs and specialist teams.

Job responsibilities

With a focus on clinical leadership and quality improvement, the post holder will: Support community teams to deliver evidence-based, pro-active individualised care; acting as a role model and enabling staff to develop confidence and competence in all aspects of generalist palliative and end of life care. Identify training needs within community teams and facilitate local learning. Lead on the implementation and ongoing review of local quality improvement initiatives. Work in collaboration with our system partners e.g. Hospices and acute Trusts to support development, implementation and ongoing review of system-wide discharge pathways relating to palliative and end of life care. The successful candidate will be a confident leader, who can influence by demonstrating excellent clinical, communication and leadership skills. They will need to work autonomously and build strong relationships across a wide range of services and teams.

Independent Non-Medical Prescribing is an essential requirement of this role and applicants must have completed this course (V300).

The role will involve direct clinical care but the post holder will not hold a clinical case load. Applicants must have experience of delivering palliative and end of life care in community settings.

Qualifications

Essential Registered first level nurse or equivalent professional registration e.g. HCPC. MSc or equivalent level knowledge/experience in relevant area. Relevant nursing/health degree. Mentor/teaching qualification. Clinical supervision training and experience. Extended/independent nurse or specialist. Experience

Essential Significant post-registration experience. Recent primary and community/nursing/therapeutic experience. Nurse-led management of minor illness, minor ailments and injuries. Nurse-led triage.
