They must be able to work independently, undertake comprehensive assessments and robust discharge plans, working collaboratively with service users, carers, and clinical teams. Possession of comprehensive IT skills are essential as there will be a requirement to provide comprehensive performance data regarding clinical activity. The successful candidate will be expected to work independently with the inpatient teams and private partners to assist in the implementation of robust and supportive discharge packages. This could mean completing referrals, assessments, presenting cases for funding to panels, attending ward reviews, and bed meetings. About us
