Band 7 Specialist Diabetes Nurse

to join our team at

Ormskirk, Lancashire . As a key member of our community healthcare team, you will have the opportunity to make a real impact on the lives of patients living with diabetes. The Diabetes Specialist Nurse service is to promote the health and wellbeing of people with diabetes enabling them to achieve optimum glycaemic control through informed self-management. The successful candidate will provide a link between GP and consultant led specialist teams and will provide education and support both to people with diabetes and those who care for them. This is a

permanent full-time role working 37.5 hours per week, Monday - Friday.

The successful candidate will be responsible for: Management of own workload, responding to urgent clinical/service needs across locality and liaising with colleagues as required. To contribute to specialist protocols and policies for diabetes. Assess, plan and implement specialist nursing care programs. This may be for individual patients or for groups of patients. Responsible for providing specialist nursing care to patients with complex needs, e.g., those patients requiring insulin therapy. About us

We are looking for a candidate who is driven, passionate and wanting to make a difference to their patients and families. You must also be: A registered general nurse (NMC) A V300 Independent Nurse prescriber (evidence: Certificate) A minimum of 2 years experience of working in the field of diabetes Experience of working in the community Full UK driving license and access to a vehicle Disclosure and Barring Service Check
