We have two available positions for this role - one on a substantive basis and the other is Maternity Cover for a 9 month fixed term contract basis. As a Case Manager in the SWPC care, the post holder will work as part ofa dynamic team in delivering an effective service to ensure safe and effective carepathways are in place for service users of specialised health care settingswithin the NHS and Independent sectors. The post holder will be required tosupport clinical staff and managers across Devon, Plymouth and Cornwall and asappropriate across the South West region and out of area. Working as part of themental health team responsible for the provision of care for specialised mentalhealth, the post holder will provide clinical support for a designated caseloadto enable the ongoing therapeutic interventions of service users in specialisedhealth care. The aim of this clinical support is to have oversight of robustaccess assessments, ensure that each service user is placed in the leastrestrictive setting with the appropriate level of supportive interventions toenable their eventual discharge into the communitywhere this is a safe option or maintain their individual needs if theyremain in a care setting. Partnership and cross boundary working Work across multiple providers within the NHS and Independent Sector tosupport delivery of successful outcomes for service users Work with all other Regional case managers in the management ofparticularly complex cases such as personality disorder and those with learningdisabilities as well as delayed discharges. Work in collaboration with services within the South West Region tomanage relationships with CCG commissioned providers that provide the referraland aftercare components of the pathway Work in collaboration with local authorities, third sector and otherstatutory bodies (such as MAPPA, Police, HMPPS, MoJ and CJS) in the managementof effective usage of the specialised mental health services. Qualifications

Registered Mental Health / Learning Disability Nurse or other registered health or social care professional Educated to masters level or equivalent level of experience of working at a senior level in specialist area. Experience

Evidence of post qualifying and continuing professional development. Must have an understanding of the background to and aims of current healthcare policy and appreciate the implications of this on engagement. Must have an appreciation of the relationship between the Department of Health and individual provider and commissioning organisations. Hold current accreditation with relevant professional body. Knowledge and Skills

Detailed knowledge of legal issues pertaining to the specialist service, including MHA 1983, after care under supervision, sex offender legislation, probation orders and life licence, Ministry of Justice procedures and child protection legislation. Extensive knowledge of one or more specialist areas, acquired through post graduate diploma or equivalent experience or training, plus further specialist knowledge or experience to masters level equivalent. Such specialist areas will be: Adult Secure - Mental Health / Learning Disability / ABI Child and Adolescent Mental Health Services (CAMHS) including secure Eating Disorders (Adults/CAMHS) Deaf Services Gender Services Extensive knowledge and experience of risk assessment and risk management. Development and maintenance of therapeutic alliance with service users and their families /cares. High-level management skills, able to perform to high level in clinical setting, holding services to account and ensuring effective use of NHSCB resources. Case management skills, focussing on the needs of the individual and the context of their pathway in relation to all clinical and legal processes. Must be able to provide and receive highly complex, sensitive or contentious information; negotiate with senior stakeholders on difficult and controversial issues, and present complex and sensitive information to large and influential groups. Ability to analyse complex facts and situations and develop a range of options. Disclosure and Barring Service Check
