Job Purpose - Monday to Friday

The service delivers high quality care to patients at in their own home with a primary purpose to prevent hospital attendance/ admission and facilitate early discharge from hospital. The service operates from 8am to 10pm, 7 days a week.

The post holder will work autonomously and in collaboration with Acute Trusts, the Adult Community Nursing Teams, other health care professionals and statutory and voluntary agencies to deliver excellent standards of patient care.

The post holder will be required to:

Work on a rotational shift basis within the Herts Unplanned Care Team - service operating hours 8am 10pm 7 day a week.

Be the clinical leader for designated shifts on a rotational basis responsible for day-to-day management of the service.

Take designated responsibility for discrete areas of operational management, line management and clinical practice.

Prioritise and manage own workload with minimal supervision.

Main duties of the job

Utilise advanced clinical and diagnostic skills and expert knowledge to carry out thorough assessments of physical and psycho-social needs in order to develop a comprehensive short term care plan aiming to optimise health and wellbeing that will prevent a hospital attendance/ admission or allow early discharge home.

Provide direct advanced, autonomous clinical care to a range of patients presenting with undifferentiated, undiagnosed and/ or complex health needs and long term conditions.

Work within Clinical Guidelines, Policies, Procedures and Patient Group Directions (PGD's) utilising expert judgment to work outside these frameworks where necessary.

Carry out specialist clinical care and treatments based on best possible evidence that will improve health outcomes. This will include cannulation, administration of intravenous infusions and antibiotics, anticoagulation monitoring, palliative care interventions and catheter and bowel management.

Deliver agreed clinical quality and performance standards within practice.

To assess referrals to the team and respond within 2 hours to ensure adequate care and support for patients and carers.

Be goal and outcome orientated, whilst maintaining a high level of performance.

Work within the principles of the Single Assessment Process.

Both verbal and written communication skills, and experience of team working will be essential to the role. Strong interpersonal skills will also be essential. You must be an NMC registered, UK based nurse to apply for this role, as it is 'zero-hours' it does not attract visa sponsorship. You must also have at least 6 months acute experience.

Person Specification

Education/Qualification

Essential

Registered Nurse with current NMC registration OR Paramedic with HCPC registration

Desirable

Evidence of continuing professional education and training at Masters Level. Further desirable qualifications in Primary Care/ Accident & Emergency/ acute clinical assessment. Qualification in teaching or practice-based learning.

Experience

Essential

Extensive experience in Primary Care and / or Accident & Emergency. Proven experience of utilising expert diagnostic and clinical skills to provide autonomous, unscheduled medical care and management of patients presenting with undifferentiated and undiagnosed conditions.

Desirable

Experience of implementing evidence- based care and developing others to do so. Change management and implementing change in practice.
