Provide skilled, effective and evidence based clinical care to patients in a variety of community settings. Use enhanced skills and knowledge to assess physical and psycho social needs and develop a comprehensive and concordant plan of care with patients, carers and the wider Multidisciplinary Team as needed. Make risk assessments, establishing the safety or otherwise of patients' ability to stay at home, or return home safely from another care setting, providing and arranging necessary nursing and supportive packages. Provide autonomous clinical care to a range of patients presenting with undifferentiated, undiagnosed and/or complex health needs and long term conditions. Support appropriate and timely case-finding of patients suitable for the service including working to support in-reach to local acute trusts as needed. Provide evidenced based clinical/therapeutic interventions based on 'best practice', in order to improve health outcomes and promote choice, e.g. Intravenous therapy, anticoagulation monitoring, palliative care interventions and catheter and bowel management.
