The Tissue Viability Team in Luton is offering a fantastic opportunity for a highly motivated and enthusiastic Band 6 to join our service.

The successful candidate must be able to demonstrate an interest and passion for Tissue Viability, have current experience with pressure ulcer/ leg ulcer management as well as leadership skills.

The role will involve supporting the Lead Tissue Viability Nurse in service development as well as reducing pressure ulceration and the management of complex wounds.

Main duties of the job

Working as part of Cambridgeshire Community Service Luton's Tissue Viability Team, the Tissue Viability Nurse will be responsible for the assessments, planning, implementation and evaluation of individualized wound care to patients in their own home, Nursing and Residential Homes and within designated leg ulcer clinics.

A key part of the role will be to co-ordinate and deliver teaching and training of pre and post registration nurses in all aspects of wound care and to be responsible for the collection, collation and analysis of statistics to identify areas of need and future service development.

In addition the role requires management and development of junior members of staff and participation in the strategic development of the Tissue Viability Service.

Job responsibilities

To be part of an integrated team delivering a high standard of patient care using an evidence based framework with a particular interest in wound care / tissue viability in community of Luton.

Provide clinical leadership and support to a team of multidisciplinary professionals providing clinical, health and social care to avoid inappropriate admission to hospital.

Be responsible for the teams assessments, planning, implementation and evaluation of individualized nursing care to patients living in the community.

Participate in the long term management of patients within the locality.

To have continued responsibility for a defined caseload of patients requiring wound care.

To be responsible for supporting the community team with the management of acute and chronic wounds as well as the prevention / treatment of pressure ulcers.

To ensure community team members are following pathways and guidelines with regards to planning, delivering and evaluating care for patients who have tissue viability issues.

To involve patients, carers, and nurses in planning of care programme and encourage self-management where appropriate through the provision of relevant teaching and support either in formal or informal teaching sessions.

To identify those patients on a caseload with complex tissue viability need that requires specialist input and to provide that support within the limits of your knowledge and expertise or refer to the tissue viability specialist nurses.

To provide up to date advice and education for patients with tissue viability problems. Ensuring all advice is evidence based and in line with trust and national policies and guidelines.

To work with and extend your knowledge and skills with the tissue viability team.

To ensure that all patients and their relatives cultural wishes are respected and their mental and physical needs are met.

To share the findings of assessment with relevant parties, formulating an individualized care plan to deliver/facilitate patient care based upon current expert practice and be responsible for referring patient when needed.

RGN Evidence of post-registration nursing experience in relevant clinical field Relevant teaching/mentoring qualification for student nurses and evidence of teaching skills

Non-medical Nurse Prescriber Evidence of Tissue Viability experience Level 6 qualification in Wound management/ leg ulcer management Leadership qualification/ training or willingness to undertake
