Approximately $22.00 - $27.00 per hour. All individual pay rates are calculated based on the candidate's experience and internal equity. Overview of Role:
Functions as a liaison and advocate for patients between their care team, specialty care, and/or other relevant staff and assists patients in navigating referrals to community resources and specialty care. Job Profile:

Bachelor's in Psychology, Social Work, Counseling, or a related degree.
5 years of experience can be substituted for a degree.
Collaborative and client-centered approach. Strong organizational skills necessary for managing client information, appointments, and follow-up items.
