These posts will have a specific focus on supporting and preparing older adolescents who are transitioning between Children's and adult services. These are new posts and are an opportunity to work with CAMHS and adult mental health services to ensure that transition between services is person-centred and needs-based and occurs in a way that supports resilience and promotes recovery.
