An exciting opportunity has arisen within the expansion of the children's crisis and home treatment team where we have a number of practitioner roles available. We believe in having a good work-life balance which is why we offer flexible working agreements.

We are looking for an enthusiastic and motivated registered professional to join our friendly multi-agency team and help us on our journey to offering outstanding care for the children and young people (CYP) within Coventry and Warwickshire. Our team consists of Mental health nurses, Paediatric nurses, Learning disability nurses and social workers and support workers.

The Crisis and home treatment model is part of the Tier 3.5 service provision which focuses on a high risk management pathway, to prevent avoidable admissions and support discharges from inpatient units. This is so CYP are in the least restrictive service for their needs. The team has been going through a transformation from the role of the Acute Liaison team to be embedded into the Crisis and Home treatment model for CYP and providing a seamless crisis response no matter where the access point is.

Main duties of the job

Your working day within the team will be dependent on the CYP's needs. You will be providing risk reviews, follow-ups for CYP who have recently had a crisis assessment, short-term intervention and referrals onto other services. You will provide mental health care to CYP and their families in line with local and national guidelines. The care you will provide will be in a range of settings such as the RISE clinics, Acute hospital wards, young people's homes or educational settings.

This post is an integral function of providing telephone support and advice to CYP up to 18 years old and their families/Carers calling 111 option two/Crisis line seeking urgent help for their mental health. You will provide information, support, signposting and advice about managing mental health. You will escalate any callers or concerns to senior clinicians.

You will provide compassionate and professional support to callers (including) carers and will support and assist CYP, families, and Carers to cope more effectively with their mental health.

You will assist callers in managing their distress and difficulties and identify immediate and non-immediate responses to 111/crisis line triage assessments.

Our team operates between the hours of 8am - 8pm Monday-Sunday where you will work staggered shift patterns which can be discussed within your interview. We have two team leads, clinical band 5, 6, 7's and support workers.

Job responsibilities

It is essential that you have professional registration such as nurse, social worker, occupational therapist, speech and language therapist or psychotherapist with experience of working with children and adolescents.

You should be able to demonstrate holistic care and risk management for the CYP and their family.

You will work within a multi-disciplinary approach within RISE and our colleagues in the acute care setting. Your role will have a key part to play in collaboratively shaping the continued development of the service model for CYP in Crisis. You will have experience within mental health and or social care work where you have undertaken assessments, formulations and risk assessments. You will be working within the trusts policies and procedures and legal frameworks such as NICE Guidelines.

You will be working collaboratively with our colleagues in the acute Trusts, Social Care, key stakeholders and GPs.

You also must be/have:

QUALIFICATIONS

Essential

Relevant Professional qualification, e.g., social worker, nurse, occupational therapist, speech & language therapist, psychotherapist, psychologist. Current Registration with relevant professional body. Significant experience working with children and adolescents Evidence of continuing professional development Professional knowledge acquired through degree/diploma
