The post offers the successful candidate a chance of caseload management in a community setting. Working one to one with patients referred into the service, the successful candidate will benefit from being part of a forward-thinking dynamic team working towards the goals and outcome measures of the commissioning pack for Cardiac Rehabilitation. The post holds many development opportunities and offers a chance to manage an autonomous caseload and plan care around patient-centered goals. Main duties of the job
The successful candidate will work the hours over 5 days, Monday to Friday, and manage a caseload of cardiac rehabilitation patients within the community setting. The service is clinic-based with occasional home visits. You will also work on the ward providing stage 0-1 of the commissioning pack pathway and in community exercise classes. We are a NACR accredited service. Job responsibilities

Registered Nurse (Level 1) Current NMC registration Relevant experience/qualification or training in CVD Relevant teaching and assessing qualification Car owner First Degree (or working towards) Independent Nurse Prescribing qualification (or willingness to work towards) Experience of working within primary care Experience of Cardio exercise programmes Cultural awareness, experience of working with ethnic minority groups Knowledge/understanding of Primary Care Experience of previous service development Disclosure and Barring Service Check
