We have an exciting opportunity for a Cardiac Rehabilitation Nurse to join the Stepping Hill Hospital Cardiac Rehabilitation Service. The post holder will work within the multi-disciplinary team flexibly, to assist in the delivery of a high-quality service, providing expert support and advice to patients with Coronary Heart Disease or post Cardiac Surgery. Main duties of the job

Provide specialist nursing assessment, advice, rehabilitation, support, information and development of evidence-based care pathways for patients following an ACS event or cardiac surgery. Be professional, legally responsible and accountable for all aspects of your own work. Ensure a high standard of clinical care, for the patients under your management, and support more junior staff to do the same. Undertake all aspects of clinical duties as an autonomous practitioner. Case manage patients and own clinical workload. Provide telephone support to patients upon discharge from acute care. Undertake a comprehensive assessment of cardiac patients by using investigative and analytical skills to formulate individual management and treatment plans. Provide counselling, goal setting on risk factor modification and interpretation of clinical information (e.g., Echocardiogram reports) to enable risk stratification for appropriate delivery of care. Supervise the Cardiac Rehabilitation exercise programme alongside the Exercise Specialist. Provide group education to patients attending the supervised classes. Communicate efficiently and effectively with the wider MDT, Consultants and GP’s. Work in line with the BACPR (British Association for Cardiovascular Prevention and Rehabilitation) Standards and Core Components. Person specification

Education

NMC Registration Evidence of highly developed specialist knowledge in the field of cardiology Specific training in coronary care or cardiology Evidence of recent CPD Knowledge

Good knowledge of Cardiology and cardiac rehabilitation Knowledge of the management and treatment of the cardiac patient, locally and nationally Knowledge of the BACPR Standards and Core Components for Cardiovascular Prevention and Rehabilitation Clinical reasoning skills Clinical audit ILS Certified & Defibrillator training Excellent knowledge of specialty Experience

Ability to work independently Use of motivational interviewing skills and assessing stage of behaviour change Working within a multidisciplinary team Working with multiple cardiac conditions, including heart failure Skills and abilities
