An exciting opportunity has arisen within Cwm Taf Morgannwg for a Band 6 Specialist Nurse to join our Cardiac Rehabilitation team at the Royal Glamorgan Hospital. We are seeking a passionate, forward-thinking individual with a background in Cardiology.

To work as part of a team of specialist nurses, providing a high standard of nursing care to all patients accessing the Cardiac Rehabilitation service. To be a key member of the multidisciplinary cardiac rehabilitation team, providing a comprehensive cardiac rehabilitation service for patients following a cardiac event. This will be in the form of education and exercise programmes to meet the individual needs of patients and may take place in a variety of settings: Hospital, clinic or community-based sessions Home visits Telephone and face-to-face support

To empower patients to self-manage their condition, by developing individual care plans and promoting opportunities for long-term self-management.

Main duties of the job Work as part of a multidisciplinary team, providing a comprehensive cardiac rehabilitation programme for patients with cardiac conditions. Receive referrals from a variety of established routes, e.g. wards, CMU, Cardiac Catheter Lab, tertiary centres, other specialist nurses, and GP's, ensuring that patients are suitable to enter the service. Demonstrate excellent communication skills when dealing with patients and relatives, delivering information in a sensitive and empathetic manner. Participate in outpatient clinics, assessing patients' clinical condition and identifying individual risk factors. Provide education and guidance in accordance with each patient's needs. Visit patients in their own home when required, ensuring that the Lone Worker policy is adhered to at all times. Produce specialist programmes of care for each individual patient, liaising with appropriate others as required, e.g. GP's, Cardiologists. Be able to analyse the findings of investigations and diagnostic testing and act in the best interest of the patient, e.g. echocardiograms, E.C.G's, and blood tests. Participate in patient education groups, providing specialist education, health promotion, and advice to patients and relatives. Provide support during exercise programmes, working closely with the specialist physiotherapist. Work closely with the Team Leader and Consultant Cardiologists discussing individual cases as required and taking advice when necessary.

Person Specification Qualifications & Experience Essential 1st level Registered Nurse Educated to Degree level and evidence of current relevant Professional Development. Experience of working / possess sound knowledge within the speciality of Cardiology. Diploma or relevant experience i.e. Cardiology / Critical Care Degree Module, Counselling, Health Promotion, teaching, and assessing. Able to work independently or within a multidisciplinary team.

Desirable Clinical History taking and Assessment Module Evidence of relevant post Cardiac course

Knowledge Essential Knowledge of national and local guidelines in relation to Cardiac Rehabilitation and CHD delivery plan in Wales.
