This is an exciting role for a kind, enthusiastic, experienced nurse who is keen to join our cardiac nurse team at Harrogate District Hospital. We are looking for someone with cardiology experience both in hospital and community. We are looking for someone who is keen, in addition to Cardiac Rehabilitation, to have an interest in supporting inpatients with heart failure and liaising closely with the community Heart Failure Specialist Nurses. To assess, plan, implement and evaluate a programme of Cardiac Rehabilitation as per NICE Guidelines and in line with BACPR standards to cardiac patients in primary and secondary care across the Harrogate and Rural District. To work independently and in conjunction with other members of the team to deliver this high-quality service to patients following cardiac surgery, ACS or PCI through face-to-face consultations and by telephone both in the hospital and in the community. To support in-patients with heart failure with often complex health needs in Harrogate District Hospital in terms of information, education and management and onward referral to community heart failure nurse team. Essential and desirable
NMC registration Educated to degree level 2 years post registration experience with some cardiology nursing experience Full driving license and use of car for community work in order to meet service needs Medical prescribing- willingness to work towards Degree modules in Cardiac rehabilitation, cardiac care and heart failure Experience of teaching and presenting at meetings Advanced knowledge of cardiac conditions Employer certification / accreditation badges
