Care Co-ordinator

for a Local Authority Client based in

North London. In this role, you will provide comprehensive assessment and care management services to support individuals with learning disabilities. About the role: You will be tasked with delivering key strategic objectives: Perform assessments, create and review care plans, and handle safeguarding cases. Work closely with service users, carers, and other professionals to coordinate care. Manage resources within budget guidelines and maintain precise documentation. Follow council policies, uphold health and safety standards, and promote inclusivity and equality. About you: The successful candidate will have the following skills / experience: Experience in conducting assessments, creating care plans, and managing safeguarding cases. Experience collaborating with multi-disciplinary teams in health and social care settings. Experience working with individuals with learning disabilities and mental health conditions. Proficiency in using information management systems and adhering to financial policies. What's on offer: Salary:

£19+ph *may negotiate higher for exceptional candidates, based on experience* Contract type:

3-6, months minimum, with a high likelihood of extension Hours:
