We are looking for an experienced Care Coordinator to join our team and support the development of a bespoke Community Care/Support Service across Buckinghamshire. The role involves coordinating individualized care services for clients, enabling them to remain in their homes and communities. Role: Care Coordinator Location: Amersham, Buckinghamshire Job Type: Full-time, Permanent Salary: £24,000-£25,000 per year What you will do: Ensure compliance with policies, procedures, and regulations. Conduct client assessments, reviews, and update care plans as needed. Liaise with families, medical professionals, and stakeholders for comprehensive support. Schedule carers, manage staff hours, and update recruitment files. Conduct staff supervisions, spot checks, and appraisals. Provide on-call cover and address punctuality or continuity issues. Promote equality, diversity, and inclusion. Deliver care in the community and transport staff if required. Identify and implement service improvements. What you will have: At least 3 years of experience in providing care (required). NVQ Level 3 Health and Social Care A valid UK Driving Licence (required). Excellent knowledge of the Buckinghamshire area. Willingness to travel (up to 75%).
