08.03.2025 Job Description: The Dene and Stour Valley PCN seeks an enthusiastic and proactive Care Coordinator to join their Wellesbourne team. The role is a new and evolving position within NHS Primary Care Networks, requiring flexibility and a good knowledge of the community or primary care sector, along with strong IT skills. Main Duties: Provide coordination and navigation for people and their carers across health and care services. Work alongside Health & Wellbeing coaches to identify patients needing additional support, especially with mental health issues. Liaise with Mind Support workers, GP’s, and Social Prescribing Link Workers. Help people manage their needs and support them in making appointments. Raise awareness of shared decision-making tools. Ensure people have quality information regarding their care choices. Assist people in understanding their engagement levels with health and wellbeing. Support access to self-management education courses. Coordinate and deliver Multi-Disciplinary Teams (MDTs) within PCNs. Manage documentation for PCN/MDT meetings. Engage in specific project work to improve patient care. Qualifications: Essential: Commitment to professional and personal development. Desirable: NVQ Level 3 or equivalent qualifications. Training in motivational coaching or equivalent experience. Experience: Essential: Experience supporting people and their families in a related role. Good experience with IT systems including EMIS Web and Docman 10. Experience in data collection and monitoring. Desirable: Experience in community development, health improvement, or supporting mental health. Experience in collaborative working across organizations.
