An exciting opportunity has arisen within the West Community Mental Health Team for a Community Psychiatric Nurse / Social Worker or OT to join us in working to provide integrated health and social care to people of working age who have moderate to severe mental health problems across a wide range of presenting conditions. The post will be based with the West Community Mental Health Team, covering Denton, Audenshaw, Dukinfield and Droylsden. There is a strong commitment to promoting your professional development. You will be supported by clinical and management supervision and will be given the opportunity to develop and enhance clinical skills, interventions and case management through a personal development and training plan. Key Responsibilities of the Post:

As a Care Co-ordinator, you will be responsible for the co-ordination, commissioning and personal delivery of individual packages of care for a caseload of service users within our secondary service. Main duties of the job:

To act as Care Co-ordinator, carry a caseload and work with evidence based interventions Assess needs Implementation of care plans Assessment and monitoring of risk Monitor, review and evaluate care Liaise with all agencies, and members of the multi-disciplinary team Carry out clinic duties such as the Clozaril / administration of depot injections either in the clients home or in the clinics if Nurse qualified. Supervise Community Support Workers / Band 5s and peers as appropriate To maintain accurate, up to date electronic records To offer initial mental health and social care assessments To participate in the CMHT’s duty system on a rota basis, offering rapid response to service users’ immediate needs, including those arising from crisis. To work in a therapeutically informed way within the multi-disciplinary team to address the needs of service users with complex emotional disorders. To maintain a persistent approach to engagement where there are complex mental health difficulties. To prioritise and work flexibly with MDT in order to respond to rapidly changing clinical needs. To promote the physical health and wellbeing To train as a Safeguarding adult’s manager if not already a SAM To complete funding applications and review placements and care of service users in a wide range of hospitals and placements. Assessment and monitoring of risk To monitor, review and evaluate care in accordance with C.P.A To liaise with all agencies, and members of the multi-disciplinary team to ensure the needs of the service user are met. Establish and maintain communication with individuals and groups on the caseload about difficult or complex matters. To carry out clinic duties such as the Clozaril / administration of depot injections either in the clients home or in the clinics if Nurse qualified. To supervise Community Support Workers / Band 5s and peers as appropriate to implement elements of the care plan. To maintain accurate, up to date electronic records of all service users referred and receiving assessment, care treatment and support. To offer initial mental health and social care assessments to those newly referred to the service. To participate in the CMHT’s duty system on a rota basis, offering rapid response to service users’ immediate needs, including those arising from crises. To work in a therapeutically informed way within the multi-disciplinary team to address the needs of service users with complex emotional disorders. To maintain a persistent approach to engagement where there are complex mental health difficulties. To have the capacity to prioritise and work flexibly with MDT in order to respond to rapidly changing clinical needs. To promote the physical health and wellbeing of service users.
