Care Coordinator

to join their supportive and dedicated team on a full-time basis. As a

Care Coordinator , you'll play a crucial role in ensuring seamless care delivery for both clients and staff. This role offers a starting salary of

£26,500 per annum , with an increase to

£28,000

upon successfully passing your probation period. In this fast-paced and rewarding role, you'll be responsible for ensuring that care is delivered efficiently and effectively, while maintaining a high standard of service for clients, staff, and the business. Key Responsibilities: Oversee staffing requirements and ensure clients are kept informed about their care schedule. Coordinate healthcare rotas, ensuring clients' needs and preferences are met as closely as possible. Allocate care calls to staff, optimizing efficiency by considering travel time, breaks, and availability. Adapt quickly to changes that may impact the rota, such as sickness, holidays, training, cancellations, or other unexpected events. Work with the team to facilitate the onboarding of new clients. Ensure client preferences are met by matching the right Care Assistant or Support Worker to each case. Be on-call for emergencies and out-of-hours service on a rotational basis, Monday to Sunday. Assist with various administrative tasks to support the healthcare team. What We're Looking For: Proficiency in MS Word and Outlook. Passionate about providing exceptional care and contributing to the health and social care sector. Strong organizational skills with the ability to prioritize tasks effectively. Willingness to learn and develop through ongoing training. Excellent team player, capable of managing and supporting others.
