The Care Coordinator will play a crucial role in supporting vulnerable individuals by coordinating care for Learning Disabilities (LD), Frail and Palliative patients, managing safeguarding concerns, and facilitating multidisciplinary team (MDT) collaboration to improve outcomes for vulnerable families and adults. You will be a team player, as well as being self-motivated, enabling you to take on your own responsibilities. You will have the opportunity to take the lead on allocated projects/services and support the team to implement, monitor and review. The post holder will work alongside the team facilitating the coordination, preparation, and documentation of support for patients in Care, Nursing and LD homes, including both virtual and in-person ward rounds. You will work alongside the Safeguarding team maintaining the organisation's safeguarding registers and searches, ensuring that the organisation is meeting its statutory obligations in terms of safeguarding to improve outcomes for vulnerable families and adults. This role combines responsibilities for Learning Disabilities, Frail and Palliative patients with safeguarding coordination, ensuring that care is compassionate, timely, and meets statutory obligations. As part of the role, you will work as a key part of the Primary Care Network (PCN) multi-disciplinary team providing care coordination as directed by the PCN Clinical Director. About us

Qualifications

Minimum GCSE (or equivalent), including Maths and English grade C Knowledge and Skills

Good organisational skills The ability to be patient, caring, calm and empathetic and can adapt to meet the needs of the patient The ability to work as a member of a team and independently Working knowledge of community services, including Palliative, Mental Health, Frailty, Learning Disabilities and EOLC Ability to act appropriately with abnormal physiological findings to have basic physical assessment skills Have working knowledge in child and adult safeguarding management, such as experience working with Adult Social Care and/or Children's services. Experience

Previous experience in General Practice or community healthcare settings. Competence with clinical systems (EMIS) and administrative tasks, including record-keeping and minute-taking. Experience of audits Community experience Disclosure and Barring Service Check

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions. £24,441 to £27,375 a year. HRH normally appoint at the bottom of the scale. Contract
